How you'll contribute

An Assessment Coordinator who excels in this role:

  • Provides clerical support to the Care Coordination Manager and Care Coordination Team and acts as pack up to front desk reception. Enters information into the Medik system accurately.
  • Maintains appropriate stock of paperwork/forms and supplies to support the Care Coordination Department.
  • Checks in patient belongings with accuracy and documents appropriately.
  • Processes consent forms and coordinates admission paperwork for unit.
  • Provides assistance to other units and departments as needed.

What we're looking for

Applicants should have a high school diploma or equivalent. Additional requirements include:

  • Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred. Must have strong clinical assessment skills.
  • Valid driver's license

  • CPR certification and Crisis Prevention Training (CPI) within 30 days

  • May be required to work flexible hours and overtime.

  • With proper credentials, Coordinator may be required to complete vitals check on patients.
