Position Summary:

In-Patient

Responsible for providing clinical nutrition services, including nutrition assessment, modified diet formulation, self-management training and nutrition intervention to patients/residents. Ensure patient satisfaction, quality care, regulatory agency compliance, and good public relations are achieved through the safe and efficient use of resources.

In-patient needs would cover inpatient diabetes education and rotating on-call for the weekends.

• Maintain high standards of existing ADA accredited diabetes program.
• Accountable for assuring diabetes instruction complies with national standards.
• In accordance with professional standards and hospital policies and procedures, responsible for conducting individualized needs assessments on persons enrolled in the diabetes and medical nutrition therapy patient education program.
• Assessment of patients and development of plans of care on all aspects of Diabetes self-management.ir medical nutrition therapy.
• Collaborating and communicating with team members.
• Contributing to and participating in a continuous quality improvement process to measure diabetes education program and to identify and address opportunities for improvement.
• Provide diabetes education and follow up to newly diagnosed and established adults with diabetes
• Education and follow up to new insulin pump patients
• Education and management of pregnant patients with Type I diabetes

Minimum Qualifications

Education:

• Bachelor's degree in nutrition and Dietetics or Registered Nurse. Language skills, Mathematical skills, Reasoning Ability.

Licensure & Certifications Requirements:

• Washington Dietitian Certification and National CDR Registered Dietitian (Commission on Dietetic Registration), Registered Nurse
