How you'll contribute
Facilitates the patient's continued recovery after discharge. Provides aftercare plan coordination for patients who are in need of ongoing medical/psychosocial care following discharge. In collaboration with other members of the healthcare team, assesses medical needs of the patient and offers suggestions regarding follow-up care options.

Qualifications and requirements:

  • Bachelor's degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.

  • Prior experience with psychiatric and chemical dependency patients.

  • Current unencumbered certification/license/registration required per state of practice guidelines.

  • Unencumbered clinical license if eligible/needed per state regulations preferred.

  • CPR certification and Crisis Prevention Training (CPI) within 30 days.
