Performs data collection, verification and transmission on patient abstracts. Reports to: Manager FLSA: Non-exempt Education: High school diploma or equivalent; Graduated from LPN program (Preferred); Associate's degree (Preferred) *Years of relevant experience may be substituted for required education. Licenses: LPN (Preferred) Certifications: Basic Life Support Health Care Provider (BLS-HCP) Minimum Work Experience 2 years related experience (Preferred) Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Essential Functions Retrieves charts from Health Information Management Department using the established records department procedures. Coordinates all aspects of the patient survey to include distribution, data entry, report generation and submission. Accurately transmits data and generates reports/graphs in accordance with the procedures in a timely manner. Maintains strict confidentiality of patient, employee and department data. Non-Essential Functions Assist with orientation and training of personnel and students. Participates and attends meetings as required and/ or assigned. Performs other related duties as assigned or requested. Prepares periodic reports as requested.

Qualifications and requirements
