The incumbent will be responsible for assessing and treating patients based on an initial evaluation/assessment.  The incumbent assures that appropriate verbal and written communication regarding the patient is documented. 


Bachelor of Health Sciences in Physical Therapy from a college or university.  Current Kentucky Board of Physical Therapy licensure and CPR required.
