We are seeking a Care Coordinator/Social Worker to join the team at Cornerstone Behavioral Health-El Dorado in Tucson!

As a Care Coordinator/Social Worker you will:

Provides support, engagement,and aftercare planning for patients receiving services within the facility.In collaboration with other members of the team, assesses needs of the patient and offers referrals and facilitates the transition to follow-up care options.Provides support with engagement activities such as program orientation, referral source engagement, treatment planning, after-care groups, psychosocial updates, engagement calls, and discharge follow up support.


Qualifications

Education: Master’s degree in social work or counseling.

Experience: A minimum of 1 year, previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and/or geriatric patients preferred. Experience coordinating care with multiple systems preferred.

License: Will require valid driver’s license and a favorable Motor Vehicle Report (MVR), along with proof of insurance when using personal or facility owned vehicle.Current Unencumbered clinical social work or counseling license preferred.
