How you'll contribute

A Clinical Liaison who excels in this role is responsible for building and maintaining referral relationships to drive appropriate admissions to the hospital. Coordinating the intake and pre-admission screening process, conducting on-site clinical assessments to determine suitability for acute rehabilitation, educating patients/families on services and participation requirements, and serving as the primary external contact for case managers, discharge planners, physicians, and payors.

  • Assisting and coordinating in-take and pre-admission screening process
  • Performing on-site (typically in referring facility) clinical judgment to determined appropriateness of patient for admission.
  • Communicating to patients and family's rehabilitation and facility options. Communicating to patients determined appropriate for admission the patient's expectations and requirements for participation
  • Securing information relating to patients' resources and benefits
  • Functioning as initial contact for external case management and discharge planner personnel with payors and hospital facilities
  • Conducting face-to-face, in-service educational visits with physicians, case managers, and other professionals.
  • Informing and educating community decision-makers about the hospital and acute rehabilitation services to develop census through direct outreach.
  • Documenting screenings, decisions, and communications accurately and timely in EMR and tracking systems.

What we're looking for

Applicants must have clinical licensure (PT, PTA, OT, OTA, SLP, RN, MSW) per state requirements. Additional requirements include:

  • Bachelor's degree preferred
  • Previous marketing/sales experience
  • Previous clinical experience preferred with demonstrated skills in clinical assessment and EMR systems
  • Valid driver's license and clean driving record
  • Excellent oral and written communication and interpersonal skills
