Position Summary:

The LPN coordinatesthe acceptance ofpatientsinto Lifepoint Health facilities. The Transfer Center Coordinator performsinitial admissionscreeningandacceptanceusingapproved criteriatoassurepatient acceptanceand admissionareconsistentwithcurrentprotocolsandprocedures.TheTransfer Center Coordinator works incollaborationwith multiple departmentswithinthehospital andfacilitates relationships with facilities staff,physicians,andnursingstaff.The Transfer Center Coordinators willworkclosely with theIntake or Placement Coordinator toidentify, trendandreportbarriers to patient throughput.

Minimum Qualifications:

Education:

• Associate's degree required
• Bachelor's degree preferred

Certifications:

• Basic Life Support (BLS) required within 30 days of hire

Experience

• Threeyearsofrelated clinical experiencein anacutecaresetting(EDor CriticalCare or Behavior HealthPreferred)

• Demonstratedskills inproblem solving, analytical andcritical thinking, prioritization, negotiation, conflict resolution, proactivedecisionmaking.

• Abilitytoestablishandmaintaincollaborativeandeffectiveworkingrelationships.

• Abilitytocommunicateeffectivelyin oral, written, andelectronicformats.

License:

• LPN licensed multistate
• For LPN's, must maintain current nursing licensure by completing applications for renewal in a timely manner and by complying with all requirements for continuing education. No nurse will be scheduled to work any shift if their nursing license has expired. It is the nurse's responsibility to ensure that the Manager of the Transfer Center Operations is notified immediately if their license status changes. Failure to comply with this requirement will result in termination of employment.
