How you'll contribute
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Mentors new staff, students, and interns.
Performs Medicaid screenings as indicated for NHP.
Assists patients with Safety Net applications when needed.
Non-Essential Functions
1. Position serves both internal co-workers and external customers, clients, patients, contractors, and vendors.
2. Access to and/or works with sensitive and/or confidential information.
3. Exhibit a comprehensive understanding of healthcare regulatory and compliance (e.g., HIPAA). Skilled in the application of policies and procedures. Knowledge of Business Office Standards and Recommended Practices.
4. Perform other duties as assigned.

Qualifications and requirements:
· The incumbent shall, at a minimum, be a Registered Nurse professional with 3-5 years clinical experience including med-surg and post-acute care settings or be a graduate of a CSWE approved master's degree program and hold applicable and current license/certification to practice. · ACSW certification is desirable
· Incumbent must hold applicable and current license/certification to practice
· Incumbent shall have demonstrated progressive responsibility in previous job roles, positive communication skills and basic computer word processing ability.
· Knowledge of reimbursement benefits and requirements including but not limited to Medicare, Medicaid and managed care is required
