The Non-Licensed Case Manager supports coordination of care, discharge planning, and communication with caregivers and the healthcare team. This role collaborates across disciplines to ensure the development and implementation of individualized care plans that meet patient needs and regulatory standards.
How you'll contribute
A Case Manager who excels in this role:
- Complete departmental orientation as well as initial and annual competencies.
- Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
- Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
- Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
- Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
- Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
- Demonstrates ability to coordinate patient care in an interdisciplinary setting.
- Familiarity with CMS documentation standards and discharge planning is preferred.
- Perform other duties as assigned.

What we're looking for
- Bachelor's or Master's degree in Social Work, or as required by state regulations (verify state-specific requirements).
- Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
- Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.
- Strong verbal and written communication skills in English; additional languages preferred. Proficiency in Microsoft Office applications required.
