Position Summary 

 

 The Clinical Pharmacist is responsible for coordinating, on a day-to-day basis, the provision of high quality, comprehensive pharmaceutical care to patients in accordance with established regulations and procedures. This includes clinical activities to enhance patient safety and quality. Some drug distribution duties will be required. Frye Regional Medical Center is dedicated to making communities healthier and being a champion of patient care.

 

Essential Functions:

  • Collaborate with physicians and other healthcare professionals to optimize medication therapy for patients. 

  • Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems including interpreting laboratory data and recommending changes as needed. 

  • Documents all clinical activities and interventions accurately and completely. 

  • Partners as an active member of multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient centered care 

  • Participating in the management of medical emergencies 

  • Provides patient counseling as appropriate 

  • Dedicates at least 50% of time on antimicrobial stewardship. 

  • Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug related problems 

  • Participates in codes 

  • Serves as a member on at least one hospital committee. 

  • Initiate or participate in quality improvement projects and recommend updates/modifications as necessary to optimize pharmacy services. 

  • Coordinate pharmacotherapy activities and education opportunities for department pharmacists and other healthcare professionals. 

  • Maintains proficiency in preceptor roles and actively emulates education/mentorship skills 

  • Maintains proficiency and actively engages in central dispensing functions and operations programs 

  • Monitors drug therapy regimens for accuracy, contraindications, duplications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug, dose, and frequency. 

  • Provides education, inservices, and training to other staff and pharmacy students as indicated. 

  • Other duties as assigned, hours scheduled, and assignments may change to meet departmental needs

  • Completes with organizational and regulatory policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards. 

  • Completes with organizational and regulatory policies for handling confidential patient information. 

  • Demonstrates excellent customer service through his/her attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization. 

  • Adheres to professional standards, hospital policies and procedures, federal, state, and local requirements and the TJC standards and or standards from other accrediting bodies

Required Skills:

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

 

Education/ Certification 

  • Bachelor of Science in Pharmacy OR Doctor of Pharmacy from an accredited College of Pharmacy (required) 

  •  Current NC pharmacy license (required) 

  •  Completion of accredited PGY1 residency program or 3-5 years of equivalent clinical experience 

  •  BCPS and Antimicrobial Stewardship certification (preferred) 

  • BLS and ACLS required within 120 days of start date
