Care Coordinator
Job Type: Full Time - Days
Schedule: Monday - Friday, 8am - 5pm

The Care Coordinator is responsible for supporting patient care continuity through timely and accurate processing of prior authorizations, primary care referral authorizations, and other clerical tasks essential to clinic operations. This role ensures patients receive coordinated access to specialty services, diagnostics, and treatments by working closely with providers, clinical staff, and external partners.

How you'll contribute:
Key Responsibilities:
- Complete and submit prior authorizations for medications, imaging, procedures, and other services as required by payers.
- Process and coordinate primary care referral authorizations to specialists and community providers.
- Track, monitor, and follow up on pending authorizations to ensure timely patient care.
- Communicate authorization statuses to providers, nursing staff, and patients as needed.
- Maintain accurate documentation within the electronic medical record (EMR).
- Assist with general clerical duties including scanning, faxing, phone calls, routing messages, and other administrative tasks as assigned.
- Collaborate with clinical and administrative teams to support smooth clinic workflow.
- Ensure compliance with payer requirements, clinic policies, and regulatory standards.
