How you'll contribute
A Discharge Coordinator who excels in this role:
Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
Creates and maintains up to date aftercare coordination community resources
Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
Provides resource information to patients and families as appropriate for future care needs.
Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.

What we're looking for
Applicants should have a Bachelor's Degree in Social Work, Counseling, Psychology, or at least three years of clinical experience within an inpatient behavioral health setting. Additional requirements include:
Prior experience with psychiatric and chemical dependency patients
Unencumbered clinical license if eligible/needed per state regulations preferred.
CPR certification and Crisis Prevention Training (CPI) within 30 days.
