Centra Behavioral Health Hospital is a brand-new 72 bed behavioral health facility opening in 2026. We offer high-quality, compassionate care for a wide variety of patients in need. Our teams provide evidence-based, transformational care for a broad range of mental health concerns in a compassionate, comfortable environment.
How you'll contribute
A Discharge Coordinator who excels in this role:
- Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
- Creates and maintains up to date aftercare coordination community resources.
- Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
- Provides resource information to patients and families as appropriate for future care needs.
- Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.
- Creates a detailed Discharge Summary and Contract for each patient as assigned.
- Works well with the interdisciplinary team including physicians, utilization review and nursing staff members. Demonstrates active communication with team members.

What we're looking for
Applicants should possess the following qualifications and skills:
- Education: Bachelor's degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
- Experience: Prior experience with psychiatric and chemical dependency patients.
- License: CPR certification and Crisis Prevention Training (CPI) within 30 days.
