How you'll contribute 

A discharge planner who excels in this role:

  • Discharge planning is the process of transitioning a patient from one level of care to the next. 

  • Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. 

  • Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care.

  • Keeps track of all Medicare forms and consent forms.

  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

  • Participates in case reviews to evaluate case management and progress. 

  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.

  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.

  • Assisting with group activities.

What we're looking for 

Applicants should have: 

  • Education: Bachelor's Degree preferred or 3 years of experience; Masters preferred 

  • Licenses: None Listed

  • Certifications: Handle With Care within 30 days of Hire
