How you'll contribute

A Licensed Practical Nurse (LPN) who excels in this role:

• Accurately performs patient assessments and identifies patient needs under RN supervision 
• Identifies and initiates appropriate nursing interventions within scope of practice 
• Provides care appropriate to condition and age of the patient 
• Performs timely and appropriate documentation relating to medical necessity in the medical record 
• Assists with completion and revision of the Interdisciplinary Care Plan for each patient 
• Performs timely and accurate QI assessments 

What we're looking for

Applicants should have a current state LPN license and possess a degree from an accredited practical nursing program. Additional requirements include:

• Graduate of an accredited licensed practical/vocational nursing program 
• Current LPN licensure in the State of New Jersey 
• Basic Life Support (BCLS/CPR) certification required 
• ACLS preferred 
• IV Certification required or must be obtained within 6 months of hire (preferred) 
• Minimum one (1) year of LPN experience; acute care or med/surg experience preferred 
• Strong communication, critical thinking, and ability to work independently 
• Passion for patient-centered rehabilitation and collaborative care 

Schedule: PRN
