How you'll contribute
A Licensed Practical Nurse (LPN) who excels in this role:
- Accurately performs patient assessments and identifies patient needs under RN supervision
- Identifies and initiates appropriate nursing interventions within scope of practice
- Provides care appropriate to condition and age of the patient
- Performs timely and appropriate documentation relating to medical necessity in the medical record
- Assists with completion and revision of the Interdisciplinary Care Plan for each patient
- Performs timely and accurate QI assessments
What we're looking for
Applicants should have a current state LPN license and possess a degree from an accredited practical nursing program. Additional requirements include:
- Graduate of an accredited licensed practical/vocational nursing program
- Current LPN licensure in the State of New Jersey
- Basic Life Support (BCLS/CPR) certification required
- ACLS preferred
- IV Certification required or must be obtained within 6 months of hire (preferred)
- Minimum one (1) year of LPN experience; acute care or med/surg experience preferred
- Strong communication, critical thinking, and ability to work independently
- Passion for patient-centered rehabilitation and collaborative care
