Licensed Practical Nurse (LPN), Post Acute, Portage Pointe

Job Type: Part-Time | 0.5 FTE or 40 hours/pp 

Schedule: 8-hour Evening/Night Shifts, Christmas Even Years, 2nd/4th Weekends

PortagePointe is a long-term care facility with 60 beds to offer the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home-like environment to residents. PortagePointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM). 

 

How you'll contribute

Post Acute LPN who excels in this role:

  • Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.

  • Documents patient care given.

  • Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family.

  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.

  • Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

  • Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

  • Works as an advocate for the physical and emotional well-being of the patient.

  • Nurtures, sustains, and protects each Elder by providing the services needed to live a full life.

  • Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment.

  • Encourages each Elder to reach and maintain his/her maximum functional ability.

  • Creates opportunities for the Elder to give care to others.

  • Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for everyone.

  • Performs other job duties as assigned.

What we're looking for

Applicants should have a state of Michigan LPN license and have graduated from an LPN or LVN Program. Additional requirements include:

  • Basic Life Support Certification

  • One-year medical surgical experience (preferred) 

  • Annual Influenza Vaccination or Declination Required
