How you'll contribute
A Patient Care Coordinator who excels in this role:
- Coordinate care and communication with internal and external providers, including medical, psychiatric, family, school, social services, and community agencies.
- Provide timely clinical updates to the treatment team based on information from families, referral sources, and community partners.
- Maintain proactive, compliant communication with patients, families, and referral sources in accordance with HIPAA requirements.
- Provide post‑discharge follow‑up, build rapport, mentor patients, and encourage continued engagement in care.
- Execute structured post‑discharge follow‑up protocols, including early transition calls, scheduled outpatient coordination, ongoing check‑ins, and crisis response support.
- Participate in crisis intervention and de‑escalation efforts to ensure patient and staff safety when needed.
- Assist patients with social determinants of health needs, including housing, transportation, insurance, and access to community resources.
- Facilitate family engagement, including providing family sessions when clinically appropriate to support communication and treatment involvement.
- Complete required clinical assessments and documentation, including psychosocial evaluations, levels of care updates, safety screenings, and assigned tools.
- Actively contribute to treatment planning, psychosocial updates, and discharge/transition planning processes.
- Provide patient and family education regarding treatment services, program expectations, and the continuum of care.
- Support transitions of care by providing referrals, resource education, and warm handoffs to outpatient and community‑based providers.
- Collaborate effectively with the interdisciplinary team, including physicians, therapists, utilization review, nursing, and clinical services.
- Communicate treatment progress and length‑of‑stay considerations to utilization review and assist with authorization or denial processes as requested.
- Complete all patient and hospital documentation accurately, timely, and in compliance with regulatory, accreditation, and organizational standards.

What we're looking for
- Education: Master's degree in Social Work, Counseling, or related field from an accredited school required.
- License: Counseling or Social Work license required.
- Minimum of one year of experience in a psychiatric or mental health setting preferred.
- CPR certification is required within 30 days of employment and prior to any patient contact.
- De-escalation certification is required within 30 days of employment and prior to any patient contact.
