How you'll contribute
A Patient Coordinator who excels in this role:
- Supports a culture of patient- and family-centered care by proactively enhancing the patient experience at the unit and departmental levels. Serves as a visible and enthusiastic change agent who helps drive meaningful action by developing action plans, goals, and objectives in collaboration with leaders, STRHS administration, staff, and physicians.
- Prepares reports, maintains filing systems, enters data, maintains databases, composes and sends correspondence, and performs other clerical functions as needed.
- Communicates and corresponds with patients, families, visitors, providers, and staff.
- Collaborates with stakeholders across the organization to achieve appropriate resolutions.
- Uses a hands-on, consultative approach that balances the organization's patient experience goals with the operational realities, limitations, and workflows of the unit or department.
- Provides professional and empathetic support to patients and families; documents, investigates, and provides feedback to appropriate leadership; and ensures complex issues are resolved within established timeframes while keeping patients and families informed throughout the process.
- Projects a positive and helpful demeanor while advocating on behalf of patients.
- Maintains patient and family confidence by safeguarding confidential information.
- Demonstrates the ability to work independently while exercising sound judgment, critical thinking, and problem-solving skills.
- Communicates effectively while maintaining a professional, calm, and empathetic demeanor when addressing concerns from internal and external customers.
- Completes rounds in Acute Care Units to communicate with patients, caregivers, and families regarding their stay.
- Conducts post-discharge follow-up calls with patients, families, and caregivers regarding the care received during their stay.
- Completes follow-up activities and notifies appropriate departmental and administrative personnel, including conducting Root Cause Analyses, regarding issues or perceived deficiencies expressed by patients, caregivers, or families.
What we're looking for
Education:
- High school diploma or equivalent required.
- Associate's degree in a healthcare related field preferred.
Certifications:
- AHA Basic Life Support (BLS)
Skills & Experience:
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action..
