How you will contribute 

A Patient Safety Officer who excels in this role:

• Leads the development, implementation, and ongoing evaluation of the hospital's patient safety program.

• Facilitates Root Cause Analyses (RCA), Learning from Defects (LFDs) and Failure Mode & Effects Analyses (FMEA).

• Oversees safety event reporting, review, triage, and follow-up, ensuring timely investigation and documentation.

• Identifies trends and system vulnerabilities, and partners with leaders to develop corrective actions and monitor outcomes.

• Ensures compliance with The Joint Commission National Patient Safety Goals, Sentinel Event Alerts, and all relevant regulatory requirements.

• Supports Lifepoint's HRO (High Reliability Organization) principles and promotes a strong culture of psychological safety.

• Provides regular patient safety reports and dashboards to the Patient Safety and Clinical Quality Committee (PSCQC), Medical Staff, and hospital leadership.

• Conducts proactive observational assessments, safety rounds, and process audits to identify operational risks.

• Educates staff on patient safety principles, error prevention techniques, and improvement methodologies.

• Collaborates with departments such as Nursing, Risk Management, Quality, Infection Prevention, Pharmacy, and Medical Staff to align safety initiatives.

• Maintains confidentiality of all investigations and patient information.

• Performs other duties as assigned to support the hospital's safety goals.

What we are looking for

Applicants should have a bachelor's degree. Master's Degree (Preferred) *Years of relevant experience may be substituted for required education. 

 

Additional requirements include:

• CPPS (Certified Professional in Patient Safety) within 2 years of hire/transfer

• 5 years of healthcare experience.
