Position Summary:
Coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use. Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department.
Essential Functions
- Manages drug therapy protocols as part of an interdisciplinary team.
- Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems.
- Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
- Documents interventions and outcomes for drug therapy quality improvement.
- Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
- Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
- Participates in performance improvement and required continuing education.
- Performs other duties as assigned.

Patient Populations Served: Neonate, Infant, Early Childhood, Late Childhood, Adolescent, Young Adult, Middle Adult, Older Adult. Protected Health Information Accessed: Demographic, Clinical, Insurance, Financial, Complete Medical Record. Exposure: Occupational exposure to bloodborne pathogens may be present. Physical Requirements (typical): Frequent standing, walking, sitting, keyboard entry, reaching, and repetitive hand/arm movements; occasional kneeling and squatting; light to moderate non-patient lifting/pushing/pulling as needed. Occasional biological and rare chemical exposures may occur.
Knowledge, Skills & Abilities
Education: Doctor of Pharmacy. License: Current license with the state Board of Pharmacy. Skills: Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments. Rapid learning and navigation of multiple pharmacy/clinical software systems. Effective English communication (read, write, speak, and understand). Ability to complete the Skills Competency Checklist within 90 days of hire.
