How you'll contribute 

• Perform and document initial and ongoing assessments of patient's condition. 

• Design and implements a plan of care which is appropriate to problems identified and involves the patient and family. 

• Educate and counsel patients and families regarding treatment plans and progress.

• Plas and/or assists with patient discharge from therapy services. Supervises activities of clinical fellows, students, and other support personnel.

Qualifications and requirements

Position Summary: Manages drug therapy protocols as a part of an interdisciplinary team. Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems. Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students. Documents interventions and their outcomes for use in drug therapy quality improvement efforts. Evaluates medication orders for accuracy and completeness prior to order entry. Checks for accuracy after filling by technicians.

Minimum Qualifications:

•  Bachelor's Degree Required; Doctoral Degree Preferred
• Licenses: Pharmacy License in the state of Washington
