The Infusion Therapy team coordinates home infusion therapy services including nursing, pharmacy, business office, patient services, purchasing, marketing and customer relations. Assesses, plans, implements and evaluates the delivery of home infusion patient care.
How you'll contribute
An RN Coordinator who excels in this role:
Accepts home infusion referrals from within and outside of the hospital and coordinates and communicates with referral sources any concerns regarding service of patients.
Communicates and collaborates with nursing, pharmacist and physician when therapy is initiated and/or changed and/or there is a change in the patient's condition.
Maintains patient charts with all required documentation necessary for clinical and business purposes.
Coordinates delivery of home infusion products and supplies.
Initiates appropriate preventative and rehabilitative nursing procedures.
Participates in patient care planning including assessments and teaching and other duties as required.
Provides departmental on-call coverage after normal business hours to respond to departmental and patient care issues.

What we're looking for
Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor's preferred).
Additional requirements include:
Valid Driver's License
3 years of nursing experience
