More about our team 

The Enhanced Supportive Care Clinic deals with outpatient palliative care patients. They see patients from all different specialties including cardiac, pulmonary, neurology, urology, and of course oncology. They deal with supportive care for patients to remain at home and avoid hospital admissions, which includes but not limited to telehealth visits, DME assistance, hospice referrals when necessary, and chronic pain control. The clinic works with the patient and family to assist with necessary items and care to reduce stress to both. The clinic also works with the inpatient palliative team to make sure that transition from inpatient to outpatient is seamless. The clinic's basic function is to provide supportive care for palliative patients due to any disease process.

 

How you'll contribute 

Registered Nurse (RN) who excels in this role:

  • Participates in patient care team's delivery of personalized, efficient care with respect for patient's spiritual and cultural differences.
  • Provides patient assessment throughout patient stay and participates in discharge planning while maintaining a therapeutic partnership with patient, family and caregivers. Assists with inpatient palliative team to transition patients to the outpatient palliative clinic
  • Initiates patient and family teaching plans focused on a patient's lifetime.
  • Assists with medication authorizations 
  • Monitor statistical reports regarding LOS and referral requests to facilitate timely care to inpatients
  • Collaborates with physicians and APPs during inpatient rounds.
  • Assist the treatment team with decision making and analyzing to resolve patient care issues or other problems.
  • Reports patient status and refers problems when necessary and anticipates situations that prevent/minimize adverse outcomes or less than optimal outcomes.
  • Follows the established plan of care and collaborates with members of the patient care team as needed. 
  • Assists physicians and non-physician providers (APPs) in completing prescription renewals electronically.
  • Documents in the electronic medical record patient vitals, updates med lists, assists in completing electronic flow sheets and documents electronically.

What we're looking for 

Applicants should have a current state RN license and possess a degree from an accredited nursing school. Additional requirements include:

  • BSN preferred

  • 1 years' experience in medical surgical nursing

  • Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS) - to be obtained within 6 months
