Pulmonary Medicine Careers: Building a Career in Respiratory and Critical Care

Pulmonologist Dr. Javardo McIntosh shares his path into pulmonary medicine, how he approaches complex respiratory conditions and why this field offers one of the most rewarding careers in critical care. In this Q&A, Dr. McIntosh covers recent advances in robotic-assisted bronchoscopy, biologic therapies and lung cancer screening and what physicians considering pulmonology and critical care should know.

From helping patients manage chronic respiratory conditions to advancing early lung cancer detection, pulmonary medicine offers physicians the opportunity to make a lasting impact on patients’ lives. For Dr. McIntosh, staff physician and core faculty member for the Pulmonary and Critical Care Fellowship Program at North Alabama Medical Center, the field combines clinical complexity, long-term patient relationships and meaningful collaboration across the healthcare continuum.

If you’re exploring pulmonary and critical care careers, Dr. McIntosh’s journey highlights the rewards of caring for patients with respiratory diseases while contributing to comprehensive, patient-centered care.

A Calling to Medicine

Like many physicians, Dr. McIntosh’s path to medicine was shaped by both personal ambition and a desire to serve others. He attended medical school at the University of the West Indies before completing his Internal Medicine residency at Wayne State University/Sinai Grace Hospital, where he served as Chief Medical Resident. He then completed a Pulmonary and Critical Care Medicine fellowship at Henry Ford Hospital in Detroit, Michigan.

His motivation stemmed from witnessing the value a physician can bring to both family and community.

"What ultimately solidified my decision was recognizing the impact that having a physician in the family can have on access to healthcare, medical knowledge and the overall quality of care."

Why Pulmonary Medicine Is So Rewarding

One of the most fulfilling aspects of pulmonary medicine is the ability to develop long-term relationships with patients while managing a diverse range of respiratory conditions. Dr. McIntosh cares for patients with illnesses such as chronic obstructive pulmonary disease (COPD), pulmonary hypertension, interstitial lung disease and lung cancer, helping them better understand and manage conditions that can significantly affect daily life.

Whether helping someone regain the ability to work and enjoy everyday activities despite COPD, identifying lung cancer at a more treatable stage or improving quality of life for patients living with chronic respiratory disease, pulmonary specialists play a critical role throughout the patient’s healthcare journey.

Advancing Patient Care Through Innovation

Pulmonary medicine continues to evolve through new technologies and treatment approaches that improve outcomes and expand access to care. According to Dr. McIntosh, several recent advancements have made a meaningful difference in patient care, including:

  • Expanded lung cancer screening programs that support earlier detection and treatment opportunities.
  • Robotic-assisted bronchoscopy and endobronchial ultrasound (EBUS), which provide less invasive methods for diagnosing and staging lung cancer.
  • Biologic therapies for select patients with severe asthma and COPD, helping reduce exacerbations, decrease corticosteroid dependence and improve quality of life.

These innovations continue to reshape pulmonary care while creating exciting opportunities for physicians interested in practicing at the forefront of respiratory medicine.

Collaboration at the Center of Care

Success in pulmonary and critical care requires close partnership across multiple specialties and disciplines. Dr. McIntosh regularly collaborates with:

  • Primary care physicians
  • Thoracic surgeons
  •  Medical and radiation oncologists
  • Radiologists and pathologists
  • Infectious disease specialists
  • Rheumatologists
