Job Summary
The Admission Coordinator screens admissions, collaborates with other departments and facilities, manages waitlist, fields questions and completes preadmission paperwork and requirements. The Admission Coordinator assists in the coordination all aspects of MDS and RAI processes by supporting the MDS Coordinator and Social Worker. Investigates, assesses, and initiates interventions to help Elder's cope with social, emotional, economic, and environmental challenges associated with living in a LTC facility.

Job Requirements
Minimum Education
Associate's Degree required
Bachelor's Degree preferred


Required Licenses: RN license required


Other Certifications: BLS required


Required Skills

• Demonstrates proficiency in verbal and written communication skills

Essential Functions
Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Performs Medicaid screenings as indicated for NHP.
Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities.
Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for each individual.
Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
Determine potential expense associated with a potential admission.
Review all applications for admission, maintain waitlist, screen referrals, act as a resource to the community.
Complete and assure the accuracy of the MDS for all residents.
Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
Maintain current working knowledge of MDS changes and most updated RAI manual.
Attends and participates in weekly IDT and QAPI meetings.
Actively involved in Family Care Conferences.
Works closely with Facility MDS Coordinator and Social Worker.
