How you'll contribute
A Case Manager who excels in this role:
Coordinates the interdisciplinary care plan, discharge planning, and patient progress
Serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

What we're looking for
Applicants should have a current license as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. Additional requirements include:
Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN)
Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred
Effective oral and written communication in English; additional languages preferred
Proficiency in Microsoft Office applications required
