How you'll contribute

A Case Manager who excels in this role:

• Coordinates the interdisciplinary care plan, discharge planning, and patient progress

• Serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.

• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.

• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.

• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

What we're looking for

Applicants should have a current license as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. Additional requirements include:

• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN)

• Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred

• Effective oral and written communication in English; additional languages preferred

• Proficiency in Microsoft Office applications required
