Who we are:

Formerly known as Towanda Memorial Hospital, this facility is a satellite campus of Guthrie Robert Packer Hospital, located 17 miles north in Sayre, Pa.

The Towanda campus joined Guthrie in April 2015, serving Bradford and Sullivan counties, offering ease of access to clinical services to those in the surrounding communities. Long-term care services are also provided for patients with a 68-bed skilled nursing unit and a 94-bed personal care home.

How you'll contribute

We're seeking a dedicated Discharge Planner who's passionate about helping individuals rebuild their lives and thrive in their daily environments. Discharge planning is the process of transitioning a patient from one level of care to the next. Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. Discharge planner reports off to team and staff.

Essential Functions
Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care
Keeps track of all Medicare forms and consent forms
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions
Assisting with group activities
Non-Essential Functions

What we're looking for- MUST BE A LICENSED SOCIAL WORKER
Education: Bachelor's Degree preferred or 3 years of experience; Masters preferred
Certifications: Handle With Care within 30 days of Hire


Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
