More about our team: Mosaic Life Care provides Inpatient Rehabilitation/Post-Acute Care in St. Joseph, MO. Therapy plays a very important role in recovery from a life-altering injury or illness. We offer a hospital-based physical rehabilitation program for the treatment of those who have experienced a debilitating injury or illness. Our Unit is a very fast-paced yet fun environment with endless opportunities to learn and grow!

What we're looking for: We are looking for a dynamic Discharge Planner who is passionate about helping others and is a team player!

What you will do in this role:

  • Discharge planning is the process of transitioning a patient from one level of care to the next. Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. Discharge planner reports off to the Social Work team and staff.

Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Essential Functions

  • Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care

  • Keeps track of all Medicare forms and consent forms

  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan

  • Participates in case reviews to evaluate case management and progress.

  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan

  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives

  • Determines, prioritizes, provides and/or arrange

Qualifications:

  • Education: High school graduate, higher education preferred.

  • Certifications: Handle With Care within 30 days of Hire.
