Essential Functions
• Assess for distress and provide emotional support to patient/family/caregiver to help cope through all phases of care: diagnosis, treatment, survivorship, and end-of-life.• Demonstrates ability to recognize, treat, monitor, and refer to appropriate resources when patient experiences distress.• Interprets patient/family/caregiver need(s) and provides information concerning the availability and limitations of resources.• Advocates and acts as a liaison on behalf of patient/family/caregiver to address barriers to care.• Evaluates and assists with transportation needs/resources for cancer treatment related appointments.• Assess for financial barriers and refer appropriately.• Communicate with staff to educate and ensure patients are properly identified and appropriately referred for psychosocial support.• Provide direct counseling interventions for patient/family/caregivers, as it relates to outpatient oncology setting.• Screens, assesses, and evaluates high-risk patients; provides crisis intervention as needed.• Provides Advanced Care Planning education to patients undergoing treatment.• May facilitate or participate in Cancer Support Group and/or other community event(s).• Documents patient interactions, interventions, and services provided in the medical record in a timely manner.• Participates in multidisciplinary conferences and meetings.• Maintains and adheres to all HIPAA guidelines and regulations.• Perform required annual training courses to maintain licensure(s).
• Act as the Psychosocial Services Coordinator for Cancer Committee.• Participates in the development and maintenance of Oncology Service Line psychosocial policies and procedures.• Knowledgeable about oncologic diseases and their treatments, psychosocial implications, and available community, national and governmental resources.

Job Requirements
Minimum Education• Master’s degree in Social Work (MSW/MSSW) from a Council on Social Work Education accredited (CSWE) program.
Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Licenses: Oncology Social Work Certification (OSW-C) preferred within 12 months of eligibility.Oncology Social Work Certification (OSW-C) preferred within 12 months of eligibility.
