Position Summary:

The Medication Reconciliation Pharmacy Technician will assist the pharmacy, nursing and medical staff in obtaining an accurate home medication list.  The technician will interact with patients and caregivers, physician offices, pharmacies, long term care facilities, and/or any other reasonable means to obtain an accurate medication list. The technician will update the medication list in the medical record to reflect accurate medications, dosages, frequencies and indications.  Care will correspond to patients’ unique medical needs, and will be appropriate to the patients age—newborn, pediatric, adolescent, adult or geriatric.  In accord with Department and institutional polices/procedure and with the rules/regulations enacted by the State Board of Pharmacy, they will support the Department’s logistic, clinical, technical and clerical functions.

Minimum Qualifications:

Required: High School graduate or equivalent. Minimum 1 year experience working in a pharmacy or administering medications in a healthcare setting. PTCB (Pharmacy Technician Certification Board) certification, or Licensed Practical Nurse

Preferred: Completion or graduate of a pharmacy technician training program or licensed practical nursing program. 3 years’ experience in either of the above areas
