This role is responsible for ensuring patients meet all quality measures throughout their care journey while working collaboratively with surgeons and the care team to deliver a seamless experience. The individual will actively promote and grow the program by educating patients on what to expect before and after surgery, including recovery expectations and self-care guidance, and may follow up with patients' post-procedure to support optimal outcomes. This position plays a key role in expanding the program's reach and impact, requiring a motivated candidate who is eager to grow, promote, and shape the program while taking it to the next level through innovation, engagement, and continuous improvement.
How you'll contribute
A RN Joint Program Coordinator who excels in this role:
Coordinates the efforts between the surgeons, leadership, and staff while tracking the progress of the program and identifying areas requiring attention.
Responsible for all program management, report generation and review, process improvement, patient and provider satisfaction.
Works collaboratively with joint arthroplasty surgeons and anesthesiologists to manage the patient's specialized needs.
Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, discharge coordination and planning, and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families.
Develops, monitors and evaluates the effectiveness of the joint replacement program care pathway and modifies as necessary.
Oversees patient education and preparation, resource case management, discharge planning and ensuring patient continuum of care services.
Identifies cost-effective measures, makes recommendations for the appropriate utilization of resources and promotes patient self-care management.
Works with the interdisciplinary teams to deliver an exceptional patient experience and monitors the quality-of-care delivery.
Evaluates and identifies patients' needs in collaboration with Joint Replacement Program Patient Navigators. When necessary, interfaces with primary care physicians, specialists, and various disciplines on the development of plan of care. Refers to and supports approved joint replacement protocols.
In conjunction with the physician, implements care/treatment plan by coordinating the episode of care for joint replacement surgery patients across multiple providers/disciplines, ensuring patient continuum of care services and monitoring outcomes.
Acts as a clinical liaison with outside care providers to include home health nursing, PT, rehab and SNFs, as well as third party administrators.
Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care. Develops individualized patient/family education plan focused on self-management; delivers patient/family education specific to joint replacement program.
Participates in data collection and analysis of clinical outcomes of care and customer satisfaction standards.
Performs other duties as assigned.

What we're looking for
Applicants should have a current state or compact state RN license and possess an associate's degree from an accredited nursing school. Additional requirements include:
Certifications: AHA Basic Life Support (BLS) required. Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) required within one year of hire.
Minimum five (5) years of clinical experience as an RN required
Background of clinical experience in Orthopedics is preferred.
Licenses: Must have a current state or compact state RN license.
Education: Associate's degree required. BSN preferred.
