Cornerstone Specialty Hospitals Conroe is a long-term acute care hospital (LTACH) located at 1500 Grand Lake Drive in Conroe, Texas. It's designed for patients who are medically complex and require extended recovery time-often following a stay in a traditional hospital ICU.

How you'll contribute

The incumbent shall provide rehabilitation/restorative nursing interventions under the supervision of the licensed nurse when a patient is discharged from formalized physical, occupational, or speech rehabilitation therapy, when a patient is admitted to the facility with restorative needs, but is not a candidate for formalized rehabilitation therapy, when a restorative need arises during the course of a custodial stay, or when a patient has restorative needs to supplement a skilled plan of care. These interventions promote the patient's ability to adapt and adjust to living as independently and safely as is possible. This concept actively focuses on achieving and maintaining optimal physical, mental, and psychosocial functioning. Restorative nursing interventions, except for use of Continuous Passive Motion (CMP), do not require a physician's order. The incumbent shall be accountable for carrying out all responsibilities in accordance with Kindred Healthcare CORE values.

ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.

  • Perform rehabilitation/restorative nursing interventions that assist or promote the patient's ability to attain his or her maximum functional potential.
  • Patient rehabilitation/restorative nursing interventions are completed as assigned.
  • Promotes patient's ability to adapt to and adjust to living as independently as possible.
  • Instructs the patient in self-care activities according to the patient's abilities.
  • Instructs the patient in the use of assistive devices for transferring, walking and mobility, dressing and grooming, eating and swallowing.
  • Provides range of motion, proper turning and positioning in bed and chair.
  • Instructs the patient on the care and use of prosthetic and orthotic devices.
  • Incorporates training and skill practice activities which include repetition, physical or verbal cueing and task segmentation.
  • Report patient response to interventions and condition to the nurse, supervising therapist(s), or assistant.
  • Documents measurable objectives and interventions in the care plan and in the clinical record.
  • Documents objective information related to the provision and patient response to nursing rehabilitation/restorative care in accordance with existing policies, procedures and guidelines.
  • Assist in formalized discipline specific and interdisciplinary group activities.
  • Reports observations and other pertinent nursing rehabilitation/restorative care information to nursing supervisor.
  • Participates in the identification of staff and patient educational needs related to nursing rehabilitation/restorative care.

Qualifications and requirements:

Education: High School diploma or general education degree (GED).

Experience: Six months experience as a Rehab Tech or Restorative Nursing Aide in a hospital or long-term care environment preferred.

Certifications: Current Basic Life Support Certification (BLS) is required. Computer and current technology competence.
