As a Discharge Planner you will:

  • Coordinates discharge planning for a specified patient population in a single hospital.

  • Follows patients throughout the continuum of care and ensures optimum utilization of resources.

  • Provides ongoing support for discharge planning through comprehensive assessment of discharge needs to include barriers to compliance with recommended plan of care post discharge. 

  • Provides support and discharge planning for patients who are in need of ongoing medical/psychosocial care following discharge. 

  • In collaboration with other members of the healthcare team, assesses psychosocial needs of the patient and offers suggestions and facilitates the transition to follow-up care options.


Qualifications

As a Discharge Planner you will have:

  • High school diploma or equivalent.

  • Bachelor’s degree preferred.

  • CPI
  • CPR
  • One year experience in a psychiatric inpatient environment.
