The Nurse Care Manager works directly with the primary charge nurse. This position provides nursing care to the adult and geriatric population and assists the unit secretary with maintaining documentation compliance for the assigned unit.

  • Serves as first point of contact for family members, residents, and staff on assigned units for issues, concerns, questions.
  • Evaluate nursing care provided by continual review of the effectiveness of the nursing care plan. Collaborates with nurses and CNA’s to make appropriate revisions to achieve the desired outcomes.

The Fauquier Health Rehabilitation and Nursing Center is a 113-bed center located on the Fauquier Hospital campus, offering nearby access to hospital and physician services. Its proximity to and affiliation with the hospital allow a rare continuity of care not found in other facilities. Services include physical, occupational, and speech rehabilitation; long term care and respite services.

Minimum Education
Licensed Practical Nurse certificate or degree required. Registered Nurse Associate's degree preferred.

Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
