Position Summary:

The Case Management Assistant supports the Case Management staff by providing clerical and discharge planning support for the department's Case Managers. These tasks include but are not limited to: arranging post-discharge services, issuing Medicare Important Message and MOON letters, providing clinical updates to
commercial payers, as well as communicating necessary discharge planning updates to the Valley View Medical Center care team staff

Reports to: The Case Management Assistant is directly accountable to the Director of the unit to which they
are assigned and ultimately to the Chief Nursing Officer.

ESSENTIAL FUNCTIONS


Assistance in discharge arrangements and hospital transfers:

  • In collaboration with the Case Manager, assists with preparation of discharges
  • Assures completeness of packet materials
  • Communicates proposed discharge plan to family and extended care facility
  • Incorporates age specific needs in discharge plan
  • Prepares timely and smooth discharges
  • Demonstrates a working understanding of federal and state regulations, departmental policies, particularly those related to discharge planning

Arranges and documents transportation services:

  • Assistance in arrangements of Home Health, Hospice, Durable Medical Equipment, OP Dialysis, OP Infusion, OP Rehabilitative Therapy and OP Wound Care services
  • In collaboration with the Case Manager, assists with arrangement of post-acute care services such as Home Health, Durable Medical Equipment, OP Dialysis, OP IV Infusion, and OP Wound Care
  • Communicates proposed arrangements to family and utilized post-acute care services
  • Incorporates age specific needs in arrangements
  • Prepares timely and smooth post-discharge plans
  • Demonstrates a working understanding of federal and state regulations, departmental policies, particularly those related to discharge planning
  • Conduct the aspects of MorCare that attribute to the clerical/support person for Case Management
    In charge of all clerical aspects of Case management at this time
  • Reviews the skilled nursing facilities requirements for admission and conveys any needs to the staff RN's
  • Contacts community resources to assist the staff RN's with discharge planning i.e. walkers, outpatient physical therapy, and home health

Assistance in issuing Medicare Important Message & MOON letters:

  • Assists in issuing Medicare Important Message and MOON letters
  • Communicates Medicare Rights to patients and their family members if applicable

Assistance in providing updates for commercial payers:

  • In collaboration with the Case Manager, assists with faxing clinical updates and to commercial payers
  • Communicates d/c date of patients to commercial payers
  • Provides updates to Case Managers & PFSD as information is received from commercial payers

Participates in and contributes to departmental, hospital, and community in-services and continuing education programs:

  • Participates in workshops and orientation processes within the department
  • Identifies community education opportunities and demonstrates initiative in collaborating with other hospital staff to participate in programs
  • Assists in designing the orientation process and serves as a preceptor to new staff members
  • Trains and mentors volunteer(s) to participate in special projects

Professional Development:

  • Maintains skills needed for performance of duties
  • Utilizes available educational opportunities for improving skills
  • Attends required in-house in-services
  • Represents department on hospital committees
  • Projects positive image of the department and hospital

Participates in the Department & Facility Performance Improvement Program:

  • Adheres to quality standards within the department
  • Adheres to established standards of work performance in all areas of assignment
  • Participates in the Performance Improvement Plan of the department as requested

Minimum Qualifications:

Education:

  • High school diploma or equivalent preferred.

Skills:

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
  • Basic experience using a computer is required.
  • Basic experience with faxing, copying, muti-phone line usage and paging required.
  • Must be able to work independently and must possess excellent verbal and written communication skills.
  • It is imperative that the Case Management Assistant has the ability to function as a team member and appropriately seek supervision as needed.
  • Must demonstrate the ability to organize and prioritize and the ability to interact with all levels of staff in a courteous and professional manner.
  • Must possess the ability to effectively function in a stressful environment and prioritize multiple assignments as appropriate.
  • Must possess the ability to use the following equipment: Telephone / Overhead Paging System / Fax Machine/ Calculator / Production copy Duplicator, & Computer

Certifications:

  • American Heart Association Basic Life Support (BLS) preferred.
