St. Francis is NOW HIRING for a Social Worker to join our Case Management team!!

Calling all MSW's, if you are looking to impact the outcome of patient discharging from the hospital and set the up for success, THIS is the position for you!

As a Social Worker on the Case Management team you will be responsible for:

  • Providing high-quality patient care
  • Complete assessment, treatment planning, group treatment, individual treatment, family treatment, and discharge planning.
  • Function as a multidisciplinary team member
  • Participate in the unit community relations program.

Qualifications

Qualifications:

  • Master’s Degree in Social Work from an institution of higher education accredited by the Council on Social Work Education Required
  • Licensed Clinical Social Worker (LCSW) Preferred
  • BLS
  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision
  • Must be able to work in a stressful environment and take appropriate action
