Position Summary:


The Social Worker is a non-exempt position that reports to the Director. This role involves investigating, assessing, and planning interventions to help patients cope with social, emotional, economic, and environmental issues. The Social Worker also serves as the Director of Social Work for Inpatient Behavioral Health accreditation purposes only. The position requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision, especially in a stressful environment where quick and appropriate action is necessary.
Essential functions of the position include interviewing and assessing patients, as well as their families, caregivers, and/or legal representatives. The Social Worker determines, prioritizes, and arranges for necessary internal and external services and interventions. Additionally, the Social Worker participates in case reviews to evaluate case management and progress, consulting with healthcare team members to promote, monitor, and evaluate compliance with the patient’s treatment plan. The role also includes assisting with discharge planning, identifying appropriate resources such as transportation, housing, healthcare, and social/spiritual services, and providing referrals as part of the discharge process. The Social Worker performs Medicaid screenings for NHP as indicated and assists patients with Safety Net applications when needed.
As Director of Social Work, the Social Worker monitors and evaluates the quality and appropriateness of social services on inpatient behavioral health units, ensuring compliance with accepted standards of practice and established policies. They conduct bio-psycho-social assessments, participate in treatment planning, and perform ongoing reassessments and evaluations. The Social Worker formulates individualized, measurable goals for patients and is involved in discharge planning, arranging follow-up care, continuity of care, and coordination with outside agencies. Utilizing various modalities, such as group and individual therapy, the Social Worker assists patients and families. Additionally, the Social Worker participates in utilization management activities to ensure proper payment for services and completes all required documentation within designated timeframes.
Non-essential functions of the role include assisting with the orientation and training of personnel and students, attending meetings as required or assigned, and performing other related duties as necessary.

Minimum Qualifications:

  • Master’s degree required
  • State of Oregon Licensed Master’s Social Worker (LMSW) required 
  • Basic Life Support Health Care Provider (BLS-HCP) required within 90 days of hire/transfer
  • Crisis Intervention Training required within 90 days of hire/transfer
