Position Summary:

To provide individualized psychotherapy, family education, and therapeutic group sessions to acute inpatient gero-psych patients aged 60 and over.  Supports discharge planning and utilization review services within a multidisciplinary treatment team in the Senior Behavioral Health Unit.

Minimum Qualifications:

  • Master of Social Work (MSW) degree required, License Certified Social Worker (LCSW) in the state of Oregon preferred.
  • Minimum 1-year internship experience, preferably in a hospital or health care setting.
  • Must have the ability to work efficiently, responsibly, and independently.
  • Excellent time management skills required.
  • Must possess strong interpersonal skills with a high sensitivity to the needs of others, including the ability to communicate effectively with staff, administrators, patients, families, physicians, volunteers, and other customers.
  • Nonviolent Crisis Intervention Training within ninety days of hire.

Essential Job Functions:

  • Complete Psychosocial Assessment, Mental Status Exam, and Alcohol Use Disorders. Identification Test within 72 hours of admission.
  • Coordinate and contribute towards individual and group therapy activities according to an established program schedule.
  • Coordinate discharge planning in conjunction with the multidisciplinary team.
  • Offer family education as needed to complete discharge coordination.
  • Participate in daily treatment team.
  • Complete utilization review in an organized and timely manner.
  • Knowledge of Community Resources and strong networking
  • Assumes responsibility for meeting annual in-service requirements and ongoing personal development
