Position Summary:
The Social Worker is a non-exempt position that reports to the Supervisor of Social Work. This role requires critical thinking skills, decisive judgment, and the ability to work independently with minimal supervision, especially in a stressful environment where prompt and appropriate action is necessary. The essential functions of the position include conducting interviews and assessments of patients, as well as their families, caregivers, and/or legal representatives. The Social Worker determines, prioritizes, provides, and arranges necessary internal and external services and interventions. Additionally, the role involves participating in case reviews to evaluate case management and progress, consulting with healthcare team members to promote, monitor, and ensure compliance with the patient’s treatment plan. The Social Worker also assists with discharge planning and processes, identifying appropriate resources such as transportation, housing, healthcare, and social or spiritual services, and providing referrals as part of the discharge plan. Furthermore, the Social Worker assesses and plans interventions to help patients cope with social, emotional, economic, and environmental problems. The role also includes supervising and training new staff, students, and interns, while providing individual, family, and group therapy as needed.
Minimum Qualifications:
- Bachelor's degree required; Masters Preferred
- Board Certified Social Worker, or related field (i.e. counseling) in the state of Washington CSW, LPCA, or LCSW required within six months of hire date.
- Handle With Care (HWC) within 30 days of hire date
