Position Summary

The Chest Pain and Stroke Coordinator ensures quality, timeliness, and effectiveness of chest pain and stroke care delivery for Person Memorial Hospital. The coordinator will oversee the cardiac and stroke data abstractions. The Patient Experience Coordinator develops, implements, and improves programs to establish and maintain Patient Experience standards or existing processes and services. The Chest Pain/Stroke/Patient Experience Coordinator will coordinate the efforts between providers, hospital administration, and staff while tracking the progress of the programs. The coordinator is responsible for all program management, report generation and review, process improvement, facilities review, budget requirements, and administrative activities of the program.

Reports to: Senior Nursing Officer

 

Responsibilities of the Position

Facilitates chest pain and stroke-related teams/task forces/committees and initiates and coordinates an in-depth analysis on near or actual misses in collaboration with risk and quality staff and develops strategies to achieve the desired results of measure sets.

Conducts chart audits/reviews for clinical practice guidelines according to current process improvement initiatives and core measure guidelines.

Employs techniques and tools associated with continuous performance improvement to assist teams in achieving goals.

Works extensively with physicians, pharmacists, nurses, and other hospital staff in achieving support for the practice of evidence-based medicine related to clinical care of chest pain and stroke patients.

Maintains an in-depth knowledge of hospital systems, patient clinical data, EMR capabilities and reports, and external databases used for public reporting and comparison.

Ensures the integrity of the data, and adheres to abstraction rules of CMS, TJC, DLP Outcomes Measurement, and Ethics and compliance policies.

May be required from time to time to perform other periodic or/duties/responsibilities, work overtime, other shifts, and/or varied schedules as requested.

Performs other functions as required or assigned.


Minimum Education

Successful completion of the Nursing Diploma Program or an Associate’s degree in Nursing from an accredited school is required.

Bachelor’s degree is preferred.

 

Certifications/Licenses

Active and valid Registered Nurse License is required.

Basic Life Support (BLS) is required through American Heart Association upon hire.

NIH Stroke Scale Certification to be obtained within 6 months of hire.

Required Skills

Critical thinking skills, decisive judgment, and the ability to work with minimal supervision.

 

Minimum Work Experience

2-3 years of recent RN experience a healthcare setting.

Skilled in Microsoft applications including Outlook, Word, Excel, and PowerPoint.
