Required Licenses Registered Nurse

Current Registered Nurse Licensure to practice in the state of Kentucky or a Current Registered Nurse Licensure with a multistate compact status.

Required Skills Certifications:

Current American Heart Association BLS certification (or) obtained within 90 days after hire date.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Language Skills: Able to communicate effectively in English, both verbally and in writing.

Minimum Work Experience

One (1) or more years of experience in an acute care setting and/or geriatric setting preferred.

Essential Functions

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Ability to formulate an individualized plan of care, revise plan as indicate by the patient's response to treatment, and evaluate overall plan daily for effectiveness.

Monitors, records, and communicates patient condition as appropriate to care team, provider, patient, and family following chain of command as appropriate.

Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.

Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

Works as an advocate for the physical and emotional well-being of the patient protecting the patient's (resident's) rights.

Documents patient/resident assessment information appropriately including documentation that is used in completing the Resident Assessment Instrument (Minimum Date Set).
