Position Summary:

Assesses and evaluates assigned patients. Conducts individual, family and/or group therapy. Functions as primary contact for treatment and discharge planning. Promotes implementation of therapeutic milieu and daily program schedule. Maintains a safe and efficient working and treatment environment per facility policies and procedures. Communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.

Essential Functions

1. Effectively performs as a treatment team member, takes and active leadership in the treatment team process.
2. Demonstrates the ability to increase awareness of existing community resources to the patient, family, and caregivers.
3. Demonstrates the ability to effectively communicate recommendations of the treatment team to the patient, family, caregivers, and other support systems.
4. Effectively interview patient, family members, and other support systems to access the bio-psycho-social status of patients scheduled for discharge. Formulates a discharge plan in a timely manner that matches patients with organizations, settings, and services best able to meet the needs of the patient. Provides ongoing assessments during hospitalization to ensure appropriate referrals based on status at the time of discharge.
5. Demonstrates the ability to integrate age-specific factors into the plan of care and involves patient and family in the care plan process.
6. Consistently conducts and documents a bio-psycho-social assessment within 72 hours of admission.
7. Facilitates groups per Program Schedule.
8. Effectively counsels patients and their families regarding psychosocial needs or problems.

Minimum Qualifications:

A high school diploma or equivalent is required
A Master's degree is required

Other: Licensed Clinical Social Worker with minimum 3 years providing care in mental health and/or substance abuse.

Required License/Certifications:
Preferred- Licensed to practice as a clinical social worker.
BLS required, or obtained during the first week of hire
Hospital-approved Behavioral Management Program will be provided.
