 This is a part time hybrid position in the Russellville, Arkansas, Benton, Arkansas ,Hot Springs, Arkansas area. Will be onsite a local clinics and flexible remote days 20-30 hrs per week. 

What you’ll do:

As an Ambulatory Care Clinical Pharmacist, you will serve as an integral member of the Clinically Integrated Network by contributing to care coordination, chronic illness management, and providing preventive care. Activities will contribute to the “Triple Aim” of health care improvement: improving the experience of care, improving the health of the population, and reducing per capita cost of health care. This is accomplished through direct patient care and medication management for patients, long-term relationships, patient advocacy, wellness and health promotion, and patient education and self-management, primarily in the adult and senior population.

Responsibilities:

Provide pharmaceutical care by performing the following:

-       participating in patient care with the medical staff during outpatient clinic visits;

-       designing, implementing and monitoring patient-specific pharmacotherapy;

-       promoting safe, evidence-based, and cost-effective use of drug therapy;

-       developing and fostering positive collaboration with providers and clinic staff;

-       documenting pharmaceutical care activities and outcomes;

-       participating in medication reconciliation when necessary;

-       following up on identified drug-related problems. 

Provide comprehensive medication management services to ambulatory care patients including, but not limited to:

-       educating patients on disease state, medication, and lifestyle management;

-       providing information, support services, and resources designed to enhance patient adherence with his or her therapeutic regimens;

-       performing Medication Therapy Management Service (MTMS);

-       addressing high risk medication use in the elderly and managing high cost and specialty medications;

Deliver Medicare Annual Wellness Visits (AWV) to eligible patients under the supervision of a physician.

Document and measure pharmacist interventions, patient outcomes, and pharmacists’ contributions to patient care.

Collaborate with internal/external team members to support the initiatives set forth by the clinically integrated network. 

Review monthly and quarterly reports on quality and cost/utilization to determine performance improvement opportunities for both CIN’s practice and its patients.

Provide effective medication and practice-related education to team members in the clinically integrated network.

Perform other duties as assigned.


What you’ll need:

Education and Licensure: Doctor of Pharmacy. Graduate of ASHP-accredited pharmacy residency program preferred. Licensed or eligible to be licensed as a pharmacist in the state of Arkansas. Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP) preferred.

 

Experience: Two years’ direct patient care clinical pharmacy practice or completion of an ASHP-accredited pharmacy residency program. Experience with Population Health program, particularly within a primary care setting or practice preferred.
