As a Team Coordinator at PortagePointe, you will have a significant impact on the lives of our Elders and their families. In this position, you will provide high quality, professional and compassionate care while providing quality oversight and supervision as delegated by the Leadership Team. Required Skills:
•    Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. 
•    Must be able to work in a stressful environment and take appropriate action.
•    Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external
•    Demonstrates proficiency in verbal and written communication skills

Position Requirements:
High school diploma or equivalent or associate’s degree required (For LPN Only) required
Current licensure as an LPN, EMT or certification as a CNA in the State of Michigan required
1 year of experience in a long term care clinical position required
3 years of experience in long term care preferred
Basic Life Support (BLS) certification required (available upon hire)
Annual influenza vaccination or declination required

Position Details:
0.8-1.0 FTE (32 to 40 hours per week)
Evening shifts - 8 Hour Shift
Every other weekend (1st and 3rd)
Christmas odd years
